Hymenotomy is a surgical procedure performed within the field of aesthetic and functional gynecology. It involves the removal or incision of a hymenal membrane with an abnormal anatomical structure. Before the hymenotomy procedure, you will undergo a mandatory consultation and examination with a gynecologist, who will:
Be prepared to provide the date of your last menstrual period and the result of your Pap smear or HPV test. During the appointment, inform your doctor about any chronic conditions, medications you are taking, and allergies. Until the day of the procedure, maintain thorough intimate hygiene, and come to the clinic wearing comfortable, loose-fitting underwear and clothing.
The procedure takes approximately 30 minutes. It is performed on an outpatient basis, in a treatment room and without the need for hospitalization, so you will return home on the same day.
STEP I: Administration of local infiltration anesthesia – you remain fully awake and conscious.
STEP II: Incision or partial excision of the hymenal tissue blocking the vaginal opening.
STEP III: Reshaping of the treated area and placement of sutures.
Remember that in the first few days after the procedure, you may experience swelling of the vulva, as well as bruising and pain in the treated area. These symptoms will gradually subside and should resolve within 7–14 days. If they occur, use cold gel compresses (from the refrigerator). Apply them to the skin of the perineum and vulva every hour for approximately 20 minutes. If you experience pain, you may also take over-the-counter painkillers as needed.
After hymenal incision, also remember to:
• keep the sterile dressing in place for 12 hours;
• protect the treated area from contamination or injury;
• maintain intimate hygiene – take gentle showers, dry the skin, and apply a wound-healing and/or anti-swelling product recommended by your doctor;
• attend all recommended follow-up appointments;
• refrain from sexual intercourse and strenuous physical activity for approximately 4 weeks.

Your doctor will rule out any possible contraindications during the pre-procedure consultation or inform you if any are present. Hymenotomy is a minimally invasive procedure, but in some cases, it may need to be postponed.
The most important contraindications include:
• active infection of the intimate area – the procedure can be performed after the infection has been treated and the inflammation has resolved;
• acute inflammatory conditions of the reproductive organs;
• uncontrolled systemic conditions that prevent the procedure from being performed;
• contraindications to the use of local anesthesia.

Indications for hymenotomy include, in particular:
• fused or thick and rigid hymenal tissue blocking the vaginal opening;
• anatomical structure of the hymenal tissue causing pain and/or discomfort;
• impaired or obstructed menstrual blood flow;
• difficulties inserting tampons or menstrual cups;
• difficulties undergoing a gynecological examination;
• difficulties with comfortable sexual intercourse.
Hymenal incision does not involve significant discomfort because the procedure is performed under local infiltration anesthesia. Mild pain may occur after the anesthesia wears off – it can be relieved with over-the-counter painkillers.
The full recovery and wound-healing period after hymenal incision or removal usually takes approximately 4 to 6 weeks.
Hymenotomy is considered a safe procedure, but like any surgical intervention, it carries a small risk of complications, such as intimate infections, swelling, or prolonged bleeding. Less commonly, wound edges may separate or sutures may dissolve prematurely, which may sometimes require another medical consultation. Some patients may also be at risk of the incised tissues rejoining too quickly, which may make it necessary to repeat the procedure.
Sexual activity can usually be resumed after approximately 4 to 6 weeks. The tissues need to heal completely. During the follow-up appointment, the doctor will assess whether it is safe to resume sexual intercourse.
